"What's wrong?" and "What's wrong with you?" mean different things!

1) What's wrong? = Are you OK? Is there a problem? - You can see we are showing concern for someone. This is a perfectly polite question to ask someone if you are worried about them.
- What's wrong Iris? You look like you've been crying.
- You look stressed Bob. What's wrong?

2) What's wrong with you? = Why did you do that bad thing? - You can see we are maybe shocked or annoyed that someone did something. We usually say this to someone to show our frustration or disappointment.
-I can't believe you invited your boss to our house for dinner without asking me! What's wrong with you?
-Stop being rude to our waiter. What's wrong with you?

So, to summarize, question 1 is polite, and question 2 should be avoided unless you are a little angry at someone 😬.

"What's wrong" is something's wrong. "What's wrong with you" is you're wrong.
