I think the description of the "eager face" comes from his love of his work. As we know, Sherlock Holmes considers anything that isn't his work as "extras," like love, and emotions in general. We only see one instance where Sherlock Holmes expresses true real concern where Watson is grazed by a bullet, but Sherlock immediately sits him down and asks him if he is okay.
