Tibia/fibula injuries are always gruesome. There are some really gruesome knee ligamentous injuries as well. The two most gruesome things I would say I’ve seen based on the amount of damage that they did were Ryan Shazier and Damar Hamlin. One caused a life-changing neurologic injury from which he is still recovering to this day. The other, the guy literally died on the field and had to be resuscitated. You can’t not include an injury where someone literally dies. Thankfully, he did not end up with any long-term sequela of injury, but in the heat of the moment his injury is the worst thing football ever had because it took his soul from his body. So basically, anything that causes a permanent neurological deficit from which you never recover full functionality or something that kills you qualify as the worst injuries imaginable.
