Do we need to be panic for Mpox outbreak?

1). Mpox virus is not a new one in humans; it is known to be circulating among humans in Africa since 1970, and thousands of suspected cases are reported in the DRC every year from 2005.

2). Mpox virus spread is not easy like SARS CoV2, because its ‘Transmissibility’, that is ‘Reproduction Number’ is lower than SARS CoV2.

3). Human-to-human transmission of Mpox is limited because the virus primarily spreads through close contact, making it harder to transmit widely.

4). Mpox is not primarily airborne, and also require a prolonged period of closeness for airborne transmission of short-range aerosols, which reduces the risk of explosive widespread transmissions even in overcrowded situations.

5). Mpox patients tend to shed the virus comparatively for a shorter period, reducing transmission opportunities.

6). The Mpox clusters are containable as they are largely confined to specific communities / population groups, allowing for effective targeted public health interventions.

7). Populations aged 50 and above may have residual immunity from previous smallpox vaccination or exposure.

8). Effective vaccines are available in preventing Mpox.
• At present, WHO recommends use of MVA-BN (Jynneos) or LC16 vaccine, or the ACAM2000 vaccine.
• Mass vaccination is not required / recommended for monkeypox now.
• Primary preventive (pre-exposure) vaccination (PPV) is recommended only for individuals at high-risk of exposure due to their behaviour or profession.
• Post-exposure preventive vaccination (PEPV) is recommended for contacts of cases ideally within four days of first exposure (and up to 14 days in the absence of symptoms)

9). Mpox is usually a self-limited disease with the symptoms lasting from 2 to 4 weeks, and the complications are comparatively rare.
 Several anti-virals, such as ‘tecovirimat’, originally developed to treat smallpox have been used to treat Mpox in exceptional circumstances, and further studies are underway.

10). The clinical picture (skin lesions) of Mpox patient is easier to identify even by paramedical personnel, and hence there is increase in chances of isolation of suspected cases and the consequent detection, and prevention of further spread of the disease.

11). Even if Mpox cases are reported in India now, there is no need for any kneejerk reaction because, since 2022, 30 Mpox cases have been reported (with the most recent case detected in March 2024), but no further explosive / connected chain of cases have been reported.

12). There are ‘post-Covid’ improvements in health infrastructure around the world including India, and Global health authorities have learned the lessons from Covid-19, and are better prepared to respond quickly and effectively to any large-scale outbreaks of any disease.


Conclusion

As per WHO, the re-emergence of Mpox might be due to a combination of:–
 Environmental and ecological changes
 Changes in animal or human movements increasing the animal-man contact
 Improvements in disease detection and diagnosis
 Cessation of smallpox vaccination since eradication in 1980
 Genetic changes in the virus due large-scale continued transmission among immune-compromised / undernourished people over a long period of time.

According to the experts, because of the stigma attached to the disease and the hectic fast mode of travels of people around the world, the non-endemic countries need to have continued vigilance and ready public health efforts in controlling the outbreak.

Nowadays, the people see the pandemic differently, and are more afraid of the stringent, restrictive regulatory control measures (like lockdowns and school closures) of the government authorities, rather than the disease itself.
